Disability Services and Accommodations
Pacific College is committed to providing equal learning access to all students, including those who qualify as persons with disabilities. While upholding this commitment, the college also expects all students to maintain the high standards of academic achievement and excellence that are essential to the integrity of the college’s mission. By advancing these aims, Pacific College ensures that its policies, practices, and procedures conform to federal, state, and local statutes and regulations.
To request accommodation for a disability, provide written documentation to Learning Support (learningsupport@pacific-college.edu) regarding the nature of your disability and any considerations/accommodations that may be necessary. Such documentation must:
(1) be from an appropriate professional,
(2) not be more than twelve months old and,
(3) provide a clear understanding of how the student is presently functioning.

The two forms included with the accommodations policy are in “fillable” PDF format. To use them:
1. Download both files first.
2. Use Adobe Acrobat or Adobe Reader to open a file.
3. You will observe that each question has a light blue box.
4. Click inside the box and type your response.
5. For longer answers, the type will get smaller as you reach the right end of the box. You can continue for a few more words, but then it will be necessary to hit the Return key to move to the next level box and continue to provide your response. This works for all wide/long responses in both forms.
6. At the end of the form, first include the date, then click on the Signature box. If you are using Adobe Acrobat, place the curser on the word SIGNATURE in the form and then right click where a dialog box will appear. Scroll cursor down and click “Sign yourself.” A dialog box will appear, and you can type your name to create a signature. Then, click “Apply” and your signature will appear in cursive. Drag your signature to the Signature box to the desired place in the form and then left click. A blue outline box will appear around your signature. Move the cursor outside the blue outline box and double left click for the signature to stay in the desired place.
7. If you are using Adobe Reader, right click inside the signature line and select “add text.” A box will appear where you can simply type in your name. Move the cursor outside the box.
8. Once you have completed the signature, the file may automatically save and close.
9. If the file saves, you can rename it with your last name in front of the form name.
10. If the file does not automatically save, click “Save As” from the file menu and a dialog box will open with the name of the file. Just click into that box in front of the name and type your last name before the other name, and then save the file.
11. The renamed file may should then be sent to the Student Services Office: learningsupport@pacific-college.edu. Alternatively, you may print the form and hand-deliver it to the Student Services Office.

Pearson VUE, the authorized centers for NCLEX-RN and NCLEX-PN exams, has compiled a list of testing clinics throughout the United States that may offer lower-cost adult assessments to help you document a disability.
The school’s confidentiality policy provides that only the appropriate school personnel access this information and it is stored in separate, confidential files. Except in instances of health or safety, information concerning the disability, accommodations, or documentation will not be released without written consent.
Provide ample time when requesting a reasonable accommodation(s). Requests must be considered and arrangements made prior to the anticipated need for service/support. Pacific College cannot guarantee that appropriate accommodations/services can be put in place without sufficient lead-time to make arrangements. Whenever possible, please provide at least sixty (60) days advance notice. The Director of Student Services or designated school official makes determinations of reasonable accommodations for students with disabilities.
